NOTE: this system is currently available for Medical Service Provider agreements only and will be rolled out gradually by department.

IMPORTANT: For each event, complete one Motivation Form. Once a Motivation Form is approved for the event you will receive an email notification; at that time complete one Intake Form for each sponsor. This system is for agreements that do not already have a designated contract management module. Unless explicitly instructed by an OGC attorney, do NOT use this system for clinical contracts, international collaboration agreements, event sponsorship agreements, equipment sale agreements, or facility use agreements: please use the module designated specifically for that agreement type.
